As Welsh hospitals continue to grapple with a wave of respiratory infections, including flu, the NHS is under significant strain. The broader healthcare system in the UK is confronting a mix of flu strains, respiratory syncytial virus (RSV), and ongoing challenges with Covid-19. Additional cases of human metapneumovirus have emerged in Wales, a virus that originated in China, causing global concerns. This multi-virus onslaught has stretched several hospitals and healthcare services to their limits.

At the beginning of the year, a distressing scene unfolded with around 15 ambulances forming a queue outside Glangwili Hospital in Carmarthen. Reports emerged of individuals being forced to remain in ambulances for extended periods, with one account mentioning a 10-hour wait. The Welsh Ambulance Service previously declared a ‘critical incident’ in late December, indicating a backlog of over 340 calls awaiting assistance. Although the situation eased after a day, it was indicative of the overwhelming demands on healthcare services.

During the peak of the flu season, approximately 281 patients were admitted to hospitals with flu-related illnesses, out of which 625 were hospitalized, with 45 patients in critical care. The latest available data for the week ending January 12, 2025, showed a reduction in confirmed cases of Covid-19, RSV, and influenza, with fewer hospitalizations compared to the previous week. Despite a decrease in confirmed cases, consultations for influenza-like illness increased during the same period.

Public Health Wales reported that 295 patients were hospitalized with confirmed Covid-19, RSV, or influenza during that week, forming 5% of all hospital admissions. Critical care admissions for acute respiratory infections also decreased, with 17 patients in critical care compared to the previous week. The total number of patients with confirmed respiratory infections in Welsh hospitals stood at 974, with 40 patients in critical care. The prominent flu strain detected was Influenza A(H1N1), while cases of Influenza A(H3N2) and Influenza B were lower.

Amid these challenges, Welsh Hospital Boards reintroduced mask mandates to mitigate infection risks on hospital premises. Hywel Dda, Cardiff and Vale University, Cwm Taf Morgannwg University, and Aneurin Bevan Health Boards enforced mask-wearing measures. While Cwm Taf Morgannwg University Health Board lifted the mandatory mask requirement due to a reduction in respiratory infections, they remain vigilant for any resurgence. Cardiff and Vale University Health Board continues to recommend masks in clinical settings, and mask-wearing is advised based on infection control needs at Hywel Dda University Health Board hospitals.

The healthcare landscape in Wales remains fluid, with fluctuations in virus cases prompting responsive actions from health authorities. As the NHS navigates through these turbulent times, the focus on infection prevention measures, including mask protocols, underscores the ongoing commitment to safeguarding public health.
